Insurance claim support

Pay at the clinic, claim with your insurer

We prepare clear English or Thai documents so you can claim physiotherapy from your own insurance. Read this page before your visit so you know what to ask your insurer.

We do not offer direct billing

You pay for your treatment at the clinic, then submit the claim to your insurer yourself. Whether physiotherapy is covered, and how much you get back, depends on your insurer and your policy. Please check with your insurer before your first session.

Documents you receive

Issued from our clinic system after payment. You can download them again any time at my.lantamed.com.

Official receipt

Itemised services and package contents, amount paid, payment method, clinic address and tax ID. Available in English or Thai.

Physiotherapy certificate

Physiotherapy assessment, treatment given, visit dates and outcome, signed by our licensed physiotherapist with their licence number.

Episode statement

One summary of every visit and bill in your course of treatment — useful when you claim several sessions at once.

Referral reference (if you have one)

If a doctor referred you, bring their medical certificate. We add the referring physician, clinic and ICD-10 code from it to your receipt and certificate. We do not issue ICD-10 codes ourselves.

What our two documents look like

Samples with fictitious data. Every real document carries a QR code your insurer can scan to check it was issued by our clinic. Click a document to enlarge.

Questions to ask your insurer

Ask these before your visit. The answers decide which documents you need.

  • Is outpatient physiotherapy covered by my policy?
  • Do I need a doctor’s referral or prescription before physiotherapy?
  • Do you need a diagnosis or ICD-10 code from a doctor?
  • Is there a limit per session, per year, or on the number of sessions?
  • Is there an excess, deductible or co-payment?
  • Do you accept a licensed physiotherapy clinic in Thailand (not a hospital)?
  • Do I need pre-approval before starting treatment?
  • Which documents, in which language, and how soon after treatment must I submit?
