Ankle Sprain: How to Recover Faster, and Why You Shouldn't Just Let It Heal on Its Own

Walking down stairs, hiking, playing sport, or simply stepping on uneven sand can all roll an ankle. Ankle sprains are among the most common injuries, and many people assume a few days' rest is all it takes.

The problem is that without full rehabilitation, a significant number of people go on to have an unstable ankle, repeated sprains, or long-term pain. That is why clinical guidelines recommend exercise-based rehabilitation after an ankle sprain [1].

When you need an X-ray or a doctor

Get checked further if:
• You cannot take 4 steps on the foot, both right after the injury and when examined
• There is tenderness over the bony bumps of the inner or outer ankle, or the outer edge of the midfoot
• The ankle looks deformed, or the foot is very numb, cold or pale
These criteria come from the Ottawa Ankle Rules, used to decide whether an X-ray is needed [1].

The first 1–3 days

The PEACE & LOVE approach to soft-tissue injuries suggests, in the early phase [2]:
• Protect: reduce weight-bearing or activities that cause a lot of pain for 1–3 days
• Elevate: raise the foot above heart level when you can
• Avoid anti-inflammatories: they may interfere with tissue healing (ask a doctor or pharmacist about medication)
• Compression: use an elastic bandage to limit swelling
• Educate: understand that the body heals, and appropriate movement matters more than complete rest

After that: LOVE

• Load: gradually return to weight-bearing and normal activity as symptoms allow
• Optimism: confidence affects recovery
• Vascularisation: pain-free aerobic exercise such as cycling or swimming
• Exercise: work on ankle range of motion, strength and balance — the part that helps prevent the next sprain [1][2]

What physiotherapy adds

A physiotherapist will assess how severe the injury is, help reduce pain and swelling, then progress balance (proprioception) and strength training step by step until you can walk on uneven ground, run or play sport with confidence. For travellers who need to keep going, we can also help plan which activities are safe during the rest of your trip.
